• Upload deed or lease agreement
• Complete and upload the appropriate residency affidavit
• Choose and upload three supplemental items such as:
- property tax bills
- mortgages
- voter registration
- vehicle registrations
- licenses
- permits
- bank statements
- utility bills
- credit card bills
- phone bills
- canceled checks (not a voided check)

Section A - Student and Parent/Guardian live together in Ramsey
Ramsey Residency Form A - must be completed by Parent/Guardian, Revised.

Ramsey Residency Affidavit A - must be completed by Landlord if there is no lease.

Section B - Student Lives in Ramsey with Someone Other Than Their Parent/Guardian
Ramsey Residency Form B - must be completed by Parent/Guardian.

Residency Affidavit B1 must be completed by Parent/Guardian. Residency Affidavit B2 must be completed by the Ramsey resident that student is living with.

Ramsey Residency Form C - must be completed by Parent/Guardian.

Residency Affidavit C must be completed by the Ramsey resident that is providing temporary housing.

Ramsey Residency Form D must be completed by the parent/guardian when the residency situation is different than what is outlined in A-C.
